Lufthansa via Frankfurt is the third viable option, and travelers lucky enough to catch their newer Allegris-configured aircraft will find one of the more thoughtfully designed business products in the sky, though Allegris rollout remains limited on US West Coast connections as of now.
My insider tip: book the Madrid-Bilbao connector with at least 90 minutes of buffer, since Iberia's regional gate assignments at T4S can require a longer walk than the connection time suggests, especially during Basque festival season in August.
